Card Reissue Request

Please fill in the information below to request a reissue of your pleasure craft operators card. Once we have received your information we will check our records to make sure you wrote the exam with us. At this time we will contact you for payment of $20 for the card reissue. Should you have not written the exam with us, we will let you know so you may find the correct institution who issued your original card.
